Conclusions
Cannabis plants produce a substantial amount of cannabinoids and other secondary metabolites. The cannabis‐based preparations can exhibit their effects by synergistic interactions between their various components, that is, changes in a bioavailability or in cellular transport, an activation or a deactivation of other metabolites, multi‐target effects of synergistic partners and other 100. It has been demonstrated that extracts of Cannabis exhibit stronger effects on the subjects with spasticity than pure THC 101. Some cannabinoids have been demonstrated to attenuate psychoactive effects of THC or smoked marijuana 13, 102. Other metabolites of Cannabis such as terpenes or flavonoids can significantly contribute to a modulation of cannabinoids’ pharmacokinetics 100. Pure cannabinoids are more convenient for study and to a subsequent standardization as a medical preparation, but still Cannabis extracts with specified amounts of cannabinoids seem to be valuable aim for further studies, also as potential anticancer agents.

An interesting idea is a combination of cannabinoids with conventional anticancer drugs, which can exhibit synergistic potential. The promising results from studies on animal models of glioblastoma treated with THC and temozolomide have led to, mentioned above, clinical trial of this chemotherapeutic agent and Sativex 94, 103. Similar observations from the study on pancreatic adenocarcinoma showed that gemcitabine administered with cannabinoids synergistically inhibited cancer cell growth 104.

To date, Cannabis or its preparations have found an application in a palliative medicine due to its analgesic and antiemetic effects, an attenuation of the side effects of chemotherapy or a capacity to treat spasticity in multiple sclerosis. Some cannabinoids‐based drugs have been registered in several countries as mentioned earlier (e.g., nabiximols, dronabinol, nabilone); however, recent meta‐analysis of the results from clinical trials involving Cannabis and cannabinoid‐based drugs demonstrates existing moderate‐ or low‐quality evidence supporting the use of these agents in a treatment. In the case of chronic pain and spasticity, there were moderate‐quality evidences supporting the use of cannabinoids. Low‐quality proofs were found to support Cannabis use in treatment of nusea/vomiting due to chemotherapy, weight loss in AIDS, sleep disorders and Tourette syndrome 14.

We are still initial stages of incorporating Cannabis products in the clinical care. There is still a lack of profound safety and efficacy clinical trials and it is very difficult or even impossible to assess the potential benefits and risk of using cannabinoids in many cases. Many aspects wait for an elucidation: the ways of administration, dosages, interactions with other drugs, or assessing adverse effects. The most common way of using recreational marijuana is smoking, which is unsuitable way of an administration from a medical point of view. Approved cannabinoids‐based drugs have form of oromucosal spray or capsules. Another important issue is the lack of easy accessible biomarkers showing the responsiveness of patients to a cannabinoid treatment.

Cannabinoids show antitumor activity in cell lines and in animal models of cancer, but we still do not have data concerning their efficacy and safety from well‐prepared clinical trials. Moreover, antitumor effects of cannabinoids have to overcome their known immunosuppressive effects which can be potentially protumorigenic. The interactions between cannabinoids and classical cytotoxic agents have to be precisely defined. These observations lead us to the conclusion, that further profound studies are doubtlessly needed to verify the idea of introducing cannabinoids into the cancer treatment.

All can be treated, claim the snake oil salesmen of the modern wild west, with the miracle cure-all: CBD, or cannabidiol. It’s one of the 119 cannabinoids contained in cannabis sativa, indica and ruderalis, and all hybrids thereof; aka weed. CBD is legal and doesn’t get you high – still-illegal cannabinoid THC does that job very efficiently – but it’s fair to say business is blazing.

What a giddy array of products there are: from CBD water (sold in clear bottles that mean the sensitive compound swiftly degrades), to cooking or massage oils, pills, chewing gum, transdermal patches, pessaries, gin, beer and lube. The crown for silliest CBD product of the year, however, belongs indisputably to the CBD-infused pillowcases sold by one hopeful firm of US fabric-makers. Yoga classes offering CBD-assisted asanas and guided meditation have sprung up, with devotees claiming greater flexibility and elevated mood.

Sellers in the UK are careful not to claim any specific medical benefits for the products because of a lack of clinical evidence, so they are instead marketed as food supplements. In this, they are supported by breathless, uncritical media reports on CBD use for airily unspecified “wellbeing” purposes.

There is now no denying the medicinal value of CBD and THC – not even by the British government, which for years maintained that lie even as it rubberstamped the cultivation and export of the world’s largest medicinal cannabis crop.

But the landmark decision in November 2018 to allow UK doctors to prescribe cannabis under extremely limited circumstances, inspired by the cases of Billy Caldwell and Alfie Dingley, whose epilepsy is improved immeasurably by medicinal cannabis products containing both THC and CBD, has left many in a limbo: knowing or believing that cannabis offers a cure, yet remaining unable to access it.

And so as media reports of miraculous cures for desperate people proliferate, the CBD industry is growing fast. Market research commissioned by the thinktank Centre for Medicinal Cannabis (CMC) estimates that the CBD market in the UK could be worth almost £1bn a year by 2025, equivalent in size to the current entire UK herbal supplement market.

And in many cases, the industry is taking consumers for a ride. Lab tests for the CMC report analysed high-street offerings and found that more than half of the most popular CBD oils sold do not contain the level of CBD promised on the label. And a look at the label of those products shows that many are sold at such low concentrations that even the guesstimated doses, measured in drops, cannot deliver more than a scant few milligrammes of the active ingredient – whereas medical trials use many times more.

Still, mislabelled or low-dose products pale into insignificance when compared to a US case reported in the journal Clinical Toxicology in April. A family unwittingly dosed their child with what was claimed to be CBD oil, but instead contained the synthetic cannabinoid receptor agonist AB-Fubinaca. This chemical is better known as an ingredient in the shortcut to oblivion otherwise known as spice.

Britain is poorly prepared for the wide-ranging changes to cannabis law that are flowering worldwide. British hemp farmers face serious commercial disadvantage as CBD may be legally extracted only from the stem and leaves of hemp crops, not from the flower, where the cannabinoids are produced in greatest profusion. Most CBD is therefore imported: a wasted opportunity to create and control – and tax – a new industry.

What is clear is that legal reform on cannabis, while welcome, is not moving anywhere near quickly enough to benefit millions of patients.

Scientists and politicians are, thankfully, catching up with hundreds of years of folk wisdom: it’s not news to anyone who regularly smokes a spliff that cannabis is relaxing, or that it can help you sleep far more soundly than a glass of red wine, or improve your mood. The interplay between THC, CBD, and the hundreds of other active compounds in the cannabis plant could one day be isolated, identified, tested and proven to offer symptomatic help or even a cure for dozens of life-threatening conditions. But decades of pointless prohibition based on specious moral arguments have prevented proper medical research that could have benefited millions.

The CBD market urgently needs proper regulation and more broadly, both the THC and CBD sectors demand the creation of a new medical model that accommodates the complexity of a plant that has been used as a medicine by humans for thousands of years.

On June 25, 2019, Florida Governor, Ron DeSantis, signed Florida Senate Bill 1020, regulating hemp and hemp extract in Florida, which includes state regulation of the popular hemp derivative, cannabidiol (CBD). The bill, which passed in May with nearly unanimous bipartisan support, makes Florida the latest state to enact legisla­tion to legalize and regulate its hemp industry, following similar action at the federal level late last year.

On December 12, 2018, President Trump signed the Farm Bill removing hemp from the list of controlled substanc­es and making it federally legal to grow and sell hemp. Hemp comes from the same cannabis plant that produces marijuana. However, marijuana has much higher lev­els of tetrahydrocannabinol (THC), the chemical in the plant that is psychoactive and generates a euphoric ef­fect. Both hemp and marijuana have CBD—a medical compound that has health benefits but is non-euphoric. Pursuant to the Farm Bill, hemp is legal as long as it contains no more than 0.3 percent THC. If hemp has more than 0.3 percent THC, it is still a federally banned controlled substance. Hemp can be transformed into a variety of products including, but not limited to, dietary supplements such as CBD oil.

In reaction to the Farm Bill, Florida has now created a framework for institution of a legalized and regulat­ed hemp industry in the state. The new law proposes to create a state hemp program within the Department of Agriculture and Consumer Services (Department) to promote the cultivation, handling, processing, and sale of hemp, hemp products, and hemp extract. Under the proposed regulatory framework, a person seeking to cultivate, handle, process, transport, or sell hemp, hemp products, or hemp extract must register with the Department. Farms or businesses that want to cultivate hemp must provide to the Department the legal land description and GPS coordinates where hemp will be cultivated.

“Hemp extract” is defined as a substance or compound intended for ingestion that is derived from or contains hemp and does not contain any other controlled substanc­es. This includes CBD. Importantly, no one can sell or distribute CBD in Florida without a certificate of analysis prepared by an independent testing laboratory that states that the hemp extract is the product of a batch tested by the laboratory and that the batch contained a total delta-9-tetrahydrocannabinol concentration that did not exceed 0.3 percent on a dry-weight basis pursuant to the testing of a random sample of the batch. The laboratory also must provide that the batch does not contain contaminants unsafe for human consumption.

Further, CBD must be distributed or sold in packaging that includes:

A scannable barcode or quick response code linked to the certificate of analysis of the hemp extract by an independent testing laboratory;
The batch number;
The internet address of a website where batch information may be obtained;
The expiration date;
The number of milligrams of hemp extract; and
A statement that the product contains a total delta-9-tetrahydrocannabinol concentration that does not exceed 0.3 percent on a dry-weight basis.
The Department, in consultation with the Florida Department of Health, is required to initiate rule-making by August 1, 2019. However, the Department is already scheduling hemp workshops all over Florida. These and other Florida hemp related information can be found on the website www.FreshfromFlorida.com.

Notably, the new law provides that every state attorney, sheriff, police officer, and other appropriate county or municipal officer shall assist with enforcing this hemp law and its administrative rules. If businesses have been selling CBD or other hemp related products prior to this law, they need to review their operations and make sure they are in compliance with this new Florida law.
